Do you handle banded versus initial-fee-plus-monthly continuation billing?
Yes. Both models are standard. Banded billing posts the full ortho fee up front and unwinds with monthly continuation claims (D8670). Initial-fee-plus-monthly bills the down payment and then bills monthly continuation through the active phase. Both are documented per office in writing and billed correctly across the full 18 to 36 month case.
How do you handle clear aligner case billing?
Initial treatment fee billed as a complete ortho code (typically D8080 or D8090) plus separate billing for replacement aligners, refinement aligners, and final retainers. Refinement aligner claims billed as treatment plan amendments where the payer policy allows. Patient AR balanced separately from insurance AR.
How do you handle multi-phase ortho (Phase 1 then Phase 2)?
Phase 1 (interceptive, typically D8030) billed during the mixed-dentition phase. Phase 2 (complete, typically D8080) billed when full adult dentition is in place. Lifetime ortho maximum tracked across both phases. Predetermination filed for Phase 2 even when Phase 1 was already approved, since payer policies often treat them as separate cases.
