Introduction:

Clinical Review is the review of patient specific clinical information including patient parameter to estimate their response to medication therapies and to detect and manage potential or actual medicine-related problems. It may also include deducing biochemical and other test assessing the patient Sign and symptoms identified from interviews with the patient and reviewing the health record.

Clinical review is a systematic review information of all examination done by clinical pharmacist on response and monitoring of drug treatment and safety aspects on daily basis.

Goals –

  1. Evaluate the response to drug treatment.
  2. Evaluate the safety of treatment regimen.
  3. Evaluate the disease progress and the need for any changes in therapy.
  4. Evaluate the need for monitoring.
  5. Evaluate the convenience of therapy to improve compliance.

Procedure –

  1. From their medication history or medication reconciliation documentation.
  2. By reviewing their health record.
  3. By discussing with other healthcare team members.
  4. By reviewing the patient clinical data and laboratory investigation.

Criteria for effective Pharmacist Clinical Review –

Pharmacist have expertise and advanced training in the effective and appropriate use of medication. Following are some important criteria for effective clinical review

  • Ensure adequate training given to person involved in clinical review.
  • Main the good working relationship with all practitioners other healthcare staff to permit clinical review of patient assess clinical report and for effective communication.
  • Identify the expert to discuss any problem and option.
  • Implement systematic approach for the clinical review and always involved patient in interrogation.
  • Decide most reliable therapeutic interventions for the individual patient.

Before any recommendations to patient or practitioner pharmacist must see the medical notes for any sensible issue related to recommendations.

Role of Pharmacist –

  • Ensure to take approval and request for clinical review of any treatment and facilitation of appropriate patient care.
  • Take approval with the clinician regarding the treatment plan and if modification need any financial coverage and for coordination with any authority. Ensure the approval notes are documented in electronic medical record of the patient.
  • Ensure the prescribed medication contribute to the best for the therapeutic outcome.
  • Assess the effectiveness of the treatment regimen status of the patient health as well as to determine the prescribed medication to optimally meet the patient need and care.
  • Identify any untreated health problem of patient and could be resolved by using appropriate medication therapy after approval from respective authority.
