WHO CAN BE NOMINATED
Nominations are only eligible if the clinic/provider does not accept Medicare, Medicaid, or any other federal or state-funded health insurance. By submitting, you confirm this restriction. -
GIFT CARD GIVEAWAY

Completing the nomination form does not guarantee a gift card.

Gift cards ($25–$50 Amazon) are awarded through a random drawing, open to all eligible entrants.

Staffingly awards no more than one gift card per individual per 12-month period.

Nothing must be purchased or paid to participate. Participants are not required to pay anything or become Staffingly clients. -
REDEMPTION PROCESS
Winners will be notified via email within 14 business days. Upon confirmation, winners receive instructions to redeem their Amazon gift card. -

By submitting, you authorize Staffingly to share your name with the nominated clinic solely to inform them of your nomination. Staffingly will not sell, rent, or share your personal contact details beyond this purpose without your permission. -

Staffingly provides administrative support services only (e.g. scheduling, insurance processing, follow-ups, prior authorizations). No medical advice or services are provided. All operations are HIPAA-compliant. -
PROGRAM MODIFICATIONS
Staffingly reserves the right to modify, suspend or terminate the program at any time for any reason (e.g. compliance changes, technical issues, low participation). -
GENERAL CONDITIONS

TAX RESPONSIBILITIES
Winners are responsible for reporting gift card value in accordance with U.S. tax law. Amazon gift cards are treated as non-cash items with value equivalent to their denomination. -
LIMITATION OF LIABILITY
Staffingly disclaims any liability for lost submissions, communication errors, or any indirect or consequential damages. Participation is at the entrant’s own risk. -

These terms are governed by the laws of your State, without regard to conflict of law principles.
